We offer non judgemental consultations and medical recommendations. Go to evs.utah.gov for more information and you can register for your state account. This is required by state of Utah they are the governing body and they issue the actual "medical Card" Once you are seen by a qualified medical provider and get your medical recommendation. You must set up an account in order to receive medical recommendation and card.
